Parents' Backpack

   The purpose of the Virtual Backpack is to provide a 'Going Green' way to get school information that would normally be handed out in paper form.  This will ultimately help the environment as well as the taxpayers who pay for paper and associated labor costs.  Thank you for your support!

Enrollment Forms Purpose
Kindergarten Enrollment Form To be submitted for all incoming kindergarten students.
Enrollment Form Grades 1-6 To be submitted for all incoming students in grades 1 through 6.
Preschool Enrollment Form To be submitted for all incoming preschool students.
Preschool physical form To be filled out by your doctor for all incoming preschool students.
K-6 physical form To be filled out by your doctor for all incoming Kindergarten - grade 6 students.
Health history form

To be submitted for all incoming students
Records release form

To be submitted to our office for a student transferring in from another school system.
Other Forms Purpose
Web & Print consent form To be filled out each year to confer (or deny) permission for a photo of your child to be put on the website, in Notable Knights, or in the newspaper.  You may submit another form during the school year to adjust your selections.
Lunch & Milk ticket form To be filled out when ordering lunch or milk coupons.
Field trip permission form Provides assurance that child is medically able to attend field trips.
