Parents' Backpack

   The purpose of the Virtual Backpack is to provide a 'Going Green' way to get school information that would normally be handed out in paper form.  This will ultimately help the environment as well as the taxpayers who pay for paper and associated labor costs.  Thank you for your support!

Enrollment Forms Purpose
Kindergarten Enrollment Form To be submitted for all incoming kindergarten students.
Enrollment Form Grades 1-6 To be submitted for all incoming students in grades 1 through 6.
Preschool Enrollment Form To be submitted for all incoming preschool students.
Preschool physical form To be filled out by your doctor for all incoming preschool students.
K-5 physical form To be filled out by your doctor for all incoming Kindergarten - grade 5 students.
6th grade physical form To be filled out by your doctor for all incoming 6th grade students.
Health history form

To be submitted for all incoming students
Records release form

To be submitted to our office for a student transferring in from another school system.
Other Forms Purpose
Web & print permission

To be submitted yearly for all students in the school. This can be resubmitted at any time to give or deny permission on a previous form.
Cafeteria news Includes the lunch ticket form and cafeteria information.
Field trip permission To be submitted yearly for all students in the school.
